Position in the team, assessment, discharge planning, case management, settings and challenges — 15 marks A. Definition and Position in the Team
The geriatric (gerontological) social worker is the professionally trained member of the interdisciplinary team responsible for the social, psychological, economic, environmental and legal dimensions of an older person's care.
Social work is one of the minimum core disciplines of every geriatric assessment programme, along with physicians, nurses and physical and occupational therapists; special geriatric expertise among team members is crucial.
The justification is structural: comprehensive geriatric assessment is multidimensional, and the interdisciplinary team conference — the critical step in the process of care — cannot set priorities without the social perspective.
In the ACE unit model, care management is deliberately shifted from a biomedical to a biopsychosocial model, focusing on the relationship between the patient and the social structures needed for effective treatment.
B. Assessment Functions
Social assessment: past social history, family relationships and social resources, living arrangements, personal preferences, finances, housing, and availability and capability of the caregiver.
Psychosocial assessment in long-term care: performed within 1–2 weeks of admission and as needed thereafter — identify serious psychosocial symptoms and refer to a mental health professional, assess discharge potential, and communicate findings to the team. Knowing the family and their preferences and concerns is critical to good nursing home care.
Evidence of independent effect: randomised trials of social worker assessment and referral in community and outpatient settings showed reduced mortality, reduced hospital use, reduced permanent nursing home use and delayed development of disability.
Safety and protection: identification of older people at high safety risk — unsafe driving, elder abuse, neglect, self-neglect, unsafe living conditions — and development of a plan for assessment or referral.
Caregiver assessment: burden, financial stressors, social isolation and loneliness, and spiritual well-being. Caregivers reporting emotional stress have significantly increased mortality, so identification of the vulnerable caregiver and referral to social work and community resources can be lifesaving.
C. Intervention and Linkage Functions
Discharge planning — the classic core role. Good discharge decisions separate two questions: (i) what type of care will best achieve this patient's goals? and (ii) which provider can best deliver that care? These require entirely different sets of information.
Planning must begin early. When started late, availability of placements trumps careful consideration of options, nursing home discharge is chosen because it is easiest to arrange, and the patient may be committed to an unnecessary lifetime of institutional care.
Matching services to eligibility: recommending home health care, day care, assisted living, rehabilitation, nursing home or hospice based on clinical needs, personal values, home circumstances, financial resources and programme eligibility.
Transitional care and information transfer: ensuring a written summary of hospital course, discharge medications, cognitive and functional status, advance directives, plan of care and follow-up reaches the patient, family and receiving providers — poor transmission of information drives readmission.
Counselling: individual and family counselling, support around placement decisions, mediation of family conflict, post-diagnostic counselling in memory clinics, and bereavement and grief support.
Resource mobilisation and advocacy: entitlements, pensions, disability certification, insurance, legal aid, guardianship, advance directives, and access to welfare schemes.
Case management: assessment → care plan → authorisation → coordination → monitoring. This matters because assessment without follow-up is unlikely to make any difference; hence "geriatric assessment" has given way to geriatric evaluation and management.
Palliative and end-of-life care: referral to a palliative care social worker promotes access to services, community resources, volunteers and transport, and supports families where the support system is already at its limit; hospice programmes provide bereavement follow-up for about a year after death.
Team, teaching and service development roles: participation in team conferences and rounds, training family caregivers, running caregiver support groups, teaching, research, and advocating for locally missing services. Social workers can obtain additional certification in gerontology.
D. Settings and Challenges
Settings: acute wards and ACE units, geriatric assessment clinics, geriatric emergency departments (case management is a listed component), day hospitals, nursing homes, community mental health teams for older people, home visits and hospices.
Challenges: great time pressure on discharge planners, shortage of trained gerontological social workers, fragmented services, meagre resources, role overlap with case managers, and the inherent conflict between being both patient advocate and gatekeeper.
Latest Updates (2023–Present)
Social connection recognised as a determinant of health
The WHO Commission on Social Connection released From Loneliness to Social Connection – Charting a Path to Healthier Societies on 30 June 2025, finding that 1 in 6 people worldwide is affected by loneliness, linked to an estimated 100 deaths every hour — more than 871,000 deaths annually.
Rates are highest in low-income countries (about 24%). The Commission determined that social connection should be treated as a "third pillar of health" alongside physical and mental health, and the World Health Assembly in May 2025 adopted its first-ever resolution on social connection as a public health priority.
In older populations, social disconnection is associated with cognitive decline and dementia, and lonely individuals are twice as likely to experience depression. Five strategic areas for action are proposed — policy, research, interventions, measurement and public engagement. This gives the geriatric social worker's core work — combating isolation and rebuilding social networks — a formal public health mandate. Structured social assessment embedded in clinical pathways
The WHO ICOPE approach is completed by the assessment and management of social care and support, covering daily activities, living conditions, finances, loneliness, elder abuse, social engagement and participation, together with a dedicated care pathway to support the caregiver — the social work domain written into a clinical algorithm.
The Age-Friendly Health Systems 4Ms framework places "What Matters" — goals-of-care conversations — at its centre, and the CMS Age Friendly Hospital Measure now makes attestation to these practices, including family caregiver involvement, a hospital-level quality expectation.
The ADS Plus multisite cluster-randomised trial (2024) across 34 adult day service sites tested a staff-delivered caregiver support programme — dementia education, support and validation, referrals and linkages, and strategies for care challenges and self-care — essentially a structured social-work intervention delivered at scale.
India — the statutory and programmatic basis
NPHCE derives from the UN Convention on the Rights of Persons with Disabilities, the National Policy on Older Persons (1999) and Section 20 of the Maintenance and Welfare of Parents and Senior Citizens Act, 2007, which deals with medical care of senior citizens. Its tertiary component, renamed Rashtriya Varishtha Jan Swasthya Yojana, runs through 19 Regional Geriatric Centres in 18 states and 2 National Centres for Ageing (AIIMS New Delhi and Madras Medical College, Chennai).
Under the MWPSC Act, 2007, a Tribunal presided over by the Revenue Divisional Officer has been constituted in each sub-division for speedy disposal of maintenance petitions — maintenance can be claimed through these tribunals or under Section 125 CrPC, with tribunals empowered to order up to ₹10,000 per month, and older people can seek eviction of abusive or neglectful children from their own property. Guiding families through these remedies is now core geriatric social work in India.
The Elder Line (national helpline 14567) provides free information, counselling, emotional support and field services for issues of care, abandonment, abuse and legal advice, while the Integrated Programme for Older Persons funds NGOs to run old age homes, day-care centres and mobile medicare units.
Economic vulnerability defines the workload: the India Ageing Report 2023 (UNFPA– IIPS) found more than 40% of elderly Indians in the poorest wealth quintile and about 18.7% living with no income, and recommended bringing all old age homes under regulatory purview and promoting in-situ ageing with day-care and elder-creche facilities.
The PM-SPECIAL scheme of the Ministry of Social Justice and Empowerment is creating a trained cadre through courses such as Geriatric Caregiver (Institutional and Home Care) and Elderly Care Companion, though an EAC-PM working paper notes that workforce development remains fragmented without a standardised certification system, and that models such as Kerala's Kudumbashree caregiver cooperatives successfully link training to employment.
